2) You agree to donate $________ cash/check/money order to the Pom Prints Rescue for services provided. Please make check/money order made payable to Lori and/or Robert Zeneski. Adoption donation is to be paid in full, no payments will be accepted.

·          All adult dogs will be spay/neutered and up to date on vaccines.

·          All puppies will require to be spay/neuter by six (6) months of age and this service will be paid for by Pom Prints Rescue.

·          The adopter must use Pom Prints Rescue’s Veterinarian ~ All About Pets Veterinary 155 East Pike Street, Cannonsburg, PA 15317 to provide the spay/neuter service, NO EXECPTIONS.

·          Pom Prints Rescue will make the appointment and will do all that is possible to have the appointment scheduled at the adopter(s) convenience, however surgeries are usually done on the 1^st Tuesday of the month. .

·          The spay/neuter appointment MUST be kept and can not be cancelled or rescheduled without good reason and Pom Prints Rescue’s approval.

·          The adopter(s) will be required to sign a spay/neuter agreement for puppies under 6 months of age.

·          The adopter(s) must provide all needed vaccines to the puppy at the age appropriate. Pom Prints Rescue will make sure the puppy is up to date on vaccines at the time of adoption to age appropriate.

·          The adopter(s) agrees to keep the dog/puppy up to date on all vaccines.  

3) Pom Prints Rescue can not predict how any dog will react in a given situation, especially without complete knowledge of its life experiences. Therefore, adoptive owners are strongly urged to exercise prudence and caution in introducing an adoptive dog into a new situation until the dog/puppy has become fully adjusted to its new environment and the adoptive owner has had an opportunity to become familiar with that dog/puppy's unique personality.

4) The dog/puppy must never be allowed to run loose. It should always carry identifying tags or have a microchip implant. Apartment dwellers must agree to always walk the dog on a leash. The dog/puppy will be kept in house in close companionship with their families.  They should also be in a home where they will be sleeping in the home and not left outdoors for long periods of time. 

 5) Adoptive owners must be prepared to demonstrate their ability and willingness to provide proper food, medical care, and a loving, safe, healthy and clean environment for the adoptive dog/puppy. Even if the dog has already had a basic obedience course, we urge a "refresher" course to establish respect and a close rapport more rapidly between dog and owner. Always be sure that the trainer is familiar with the dog/puppy's temperament.

 6) The ownership and responsibility for the adopted dog/puppy belongs solely to the adoptive owners. However, if for some valid reason, the adoption does not work to the advantage of the new owner and the dog, Pom Prints Rescue must be contacted and given the option of reacquiring the dog/puppy before any further placement. The adoptive owner shall not transfer ownership of the dog/puppy. I understand the adoption donation is non-refundable and the dog/puppy must be surrendered back to Pom Prints Rescue.

 7) Inasmuch as is practical, each prospective adoptive home will be visited by a Pom Prints Rescue member to evaluate its qualifications as an adoptive home.

 8) The Pom Prints Rescue makes no representations or warranties regarding the temperament, health or physical condition of any adoptive dog/puppy. All warranties, including implied warranties of merchantability and fitness for a particular purpose, where they are applicable, are hereby specifically disclaimed.

 9) The Pom Prints Rescue shall not be responsible for any bodily injuries or property damage caused by any adopted dog/puppy or by the actions of its adoptive owners. The adoptive owner hereby specifically assumes sole responsibility for, and agrees to hold the Pom Prints Rescue harmless from, any and all loss and expenses (including legal fees) by reason of liability imposed by law upon Pom Prints Rescue, or any of it's officers and members because of bodily injuries, or any damage to property arising out of or in consequence of the placement of the dog/puppy, howsoever such injuries, death or damage to property may be caused, whether or not the same may have been caused, or may be alleged to have been accused, by negligence of the aforementioned parties or any of their employees, agents or members, or any other person.

10) You the adopter(s) agree to keep Pom Prints Rescue informed of the current home address and phone number.

Violation of the terms above will constitute breech of agreement and Pom Prints Rescue will remove the dog/puppy from the adopter(s) care and the donation is non refundable. If breech of agreement occurs and the dog/puppy is not surrendered voluntarily back to Pom Prints Rescue we will file in court for the return of the dog/puppy. The adopter(s) will be responsible for all court charges.

 I understand and agree by signing this agreement the adoption becomes final and the donation is non refundable.
